We must have your original, signed Volunteer Liability Form in order for you to volunteer. For insurance purposes, there can be no exceptions! A link to the Form (downloadable PDF) is shown below.
If it is more than one week to the date you are volunteering, please sign the Form and mail it to:

If you will be volunteering in less than a week, please bring the signed Form with you and give it to your Volunteer Coordinator. Thank you!
If you forget the bring the Form with you, your Volunteer Coordinator should have blank Forms available.
